NOSE REFINEMENT SURGERY

Rhinoplasty, nose surgery, will transform your face, but if you dislike your nose there are certain rules concerning what results the surgery can achieve.

A very large nose cannot be chiselled down to a very small one, it is best to think of it as one or two sizes different, although humps, tips and nostrils can be changed without any problem. Noses can go up in size too with the help of extra cartilage or a silicone implant although this is less common.

Noses grow during adolescence and stop around the age of 18 so there is no point having nose surgery until then. It is important to remember that there is no such thing as the perfect or normal nose and no noses are symmetrical. We all have different shaped noses and often they are in harmony with the rest of our faces so it may only be our personal vanity that make up dissatisfied with our noses.

Noses defend us against pollution and the nose is our organ of smell.  Damage to the nose can affect the airway and cause irritation, but noses were designed to have a job to do as well as having to look good.

Nose refinement surgery is for people who want a different nose to their own uninjured one or a replacement to an ugly or ethnic nose. Sometimes an adjustment is needed to the tip of a drooping nose, possibly caused by old age when the fat is absorbed and the skin thins so the cartilage sticks out.

Your surgeon will want to take photo's of your own nose before the surgery and today's' technology enables them to put your face on the computer and you can try out different noses, by virtual image, until you find the one you like. The surgeon will discuss the possibilities of the type of nose you want, as it also depends on what the surgeon thinks is aesthetically and technically realistic for your face.

There are three types of nose refinement surgery:

Rhinoplasty

This operation is done from inside the nose so there is no external visible scar. This operation is suitable if the nose is already straight and there is no problem with the tip of the nose.

External rhinoplasty

This operation involves cutting the nose at the strip of tissue separating the nostrils. This bit of tissue inside the nose is an area that is hidden so there are no external scars. If the nose is narrowed the scars will be hidden in the crease where the nose joins the cheek.

Tip surgery

This is not a full rhinoplasty operation. If the surgeon needs to take away some bone, then it is easy to remove, but if cartilage needs to be removed it is more difficult because cartilage is springy and often springs back to its original position.

The nose refinement operation involves breaking the nose and pushing both sides in. The break can be done from inside the nose or by inserting a chisel through tiny nicks in the skin between the eyes or on the cheek. These tiny holes, heal up quickly without stitches and are scar free.

Most operations involve adjusting the tip first and reducing the nose in line with the tip by cutting the bone.

Rhinoplasty operations take 1-2 hours under general anaesthetic with an overnight stay in hospital. When you wake up your nose will be covered in plaster of Paris or a plastic splint taped over the nose and ice packs will be given to reduce the swelling around the eyes. There is normally very little discomfort after surgery but you must avoid sneezing or blowing your nose.

Stitches are taken out in 4 days and the plaster removed after about 10 days. The nose will feel numb for a while and the swelling may take several months to go away. Numbness will take longer, maybe 6 months, and during this time it is important to avoid heat and sun exposure.

Most people are very pleased with their new nose but occasionally surgeons produce a nose that the patient doesn't like and around 1 in 20  rhinoplasty patients want another operation because they are not happy with it.
